• Coordinate patient care with care team and navigate patients through the health care system.
• Ensure patients are appropriately scheduled for clinical needs and connected to clinical, social, and community resources.
• Provide emotional support and education to patients and families within the specialty care discipline.
• Collaborate with the health care team to coordinate specialty care needs and medical management.
