Qualifications

• Education: Bachelor's degree in social work, Family and Human Development, Public Health, or related field.
• Experience: Minimum of 3 years’ experience working with families and community systems; may substitute for degree.
• Knowledge/Skills: Excellent written and verbal communication; ability to work with diverse families and teams.
• Technical: Computer skills and data entry into program databases.

Responsibilities

• Provide comprehensive care coordination services to children and families by linking to medical, developmental, behavioral, educational, and social services.
• Collaborate with families and providers to develop and implement a shared plan of care.
• Partner with inpatient, outpatient, and behavioral health teams to ensure smooth transitions of care.
