Responsibilities

• Collaborate with the health care team to coordinate cost-effective services aligned with the member's plan of care.
• Complete psychosocial assessments to evaluate goals, support systems, resources, and barriers.
• Develop and monitor a plan of care with the nurse case manager to promote optimal functioning.
• Identify and refer to appropriate resources to meet the care plan in a cost-efficient manner.
• Document case management activities per organizational standards.
• Analyze outcomes to identify improvements in program quality and cost effectiveness.
• Assist with applying for local, state, and federal programs and refer as needed.
• Provide counseling and education to members, families, caregivers, and staff.
