Ideal candidates have a high school diploma or Associate degree and at least two years in a subspecialty program, with strong customer service and leadership skills.

Qualifications

• High school diploma or equivalent; an Associate degree in Health, Business and /or related field preferred.
• At least 2 years' experience within a specified subspecialty program (e.g., Liver/Heart transplant, Heart failure) is expected.

Responsibilities

• Performs all functions of the Care Coord/Auth/Sched Specialist.
• Serves as a central point of contact for the program for patients and staff.
• Initiates and facilitates resolution of patient issues and complaints with others as needed.
• Coordinates program meetings and committees such as Selection and group teaching.
• Completes data entry or data collection for program quality initiatives.
• Audits documentation to ensure regulatory compliance.
• Orders program-specific medical supplies.
• Participates in department-wide committees and cross-functional quality projects.
• Precepts and mentors new hires.
• Ensures clerical regulatory requirements of the program are met.
• Performs provider schedule maintenance and coordination of care team meetings.
• Maintains regulatory and quality tracking spreadsheets.
• Processes payments for program invoices and expenses.
• Participates in program committees, councils, projects, and events.
• Schedules meetings for management and team.
