Central Peninsula Hospital is seeking a PCMH Care Coordinator for our new Patient-Centered Medical Home program in Soldotna. This full-time, day-shift role supports patient outreach, navigation, referral tracking, and administrative follow-through for the centralized PCMH team.

Ideal candidate has an associate degree in a related field and at least one year of relevant healthcare office or patient support experience. Grant-funded position with comprehensive benefits.

Qualifications

• Associate's degree required in a related field.
• Relevant experience may be accepted in lieu of education on a year-for-year basis
• 1 year of relevant technical experience in healthcare office, patient access, community services, or a related patient support role

Responsibilities

• Provide patient outreach, navigation, and referral tracking for the PCMH program.
• Handle administrative follow-through for the centralized PCMH team.
• Support an assigned work queue and connect patients with the appropriate care team member.
