• Lead phone interviews to gather pertinent information to initiate service referrals and change requests.
• Determine preliminary MO HealthNet and HCBS programmatic eligibility.
• Review, monitor, and update information in various electronic systems and databases.
• Coordinate with a blended team of professionals to ensure quality care.
• Provide objective and accurate documentation.
• Classify and generate actions for multiple HCBS teams.
• Serve with exceptional professionalism in a contact center environment.

This position will be located at a base location that is flexible within the State of Missouri. This position is telecommuting-eligible if candidate meets all telecommuting eligibilities and qualifications. .
