Qualifications

• High School diploma or GED required.
• Associate degree or equivalent work experience preferred.
• CPR certification is required/preferred.

Responsibilities

• Lead and supervise Personal Care Attendants (PCA) delivering ADL/IADL services.
• Coordinate schedules, training, and evaluations for PCAs.
• Visit participant homes to assess PCA performance and provide orientation.
• Collaborate with IDT to create and monitor care plans.
• Attend care planning and IDT meetings and report to families.
