Job summary

Neighborhood Healthcare PACE seeks a CNA/PCA to support personal care, companionship, and escort services for participants in the PACE program. The role covers ADLs, meals, and attending medical appointments, with a Monday–Friday 8am–5pm schedule.

Qualifications include a High School diploma or GED, active CA CNA certification, and BLS/CPR/First Aid certifications. Prior experience with frail elderly populations is preferred.

Qualifications

• Minimum six months experience as a Personal Care Aide.
• Minimum one year experience with frail elderly population.

Responsibilities

• Performs and records vital signs and other routine tasks.
• Assists with admissions, transfers and discharges.
• Provides personal care (toileting, bathing, grooming).
• Escorts participants to medical appointments and pharmacy visits.
• Assists with meals, activity programs and daily living activities.
