Job summary

AltaMed Health Services Corp. is seeking a Personal Care Attendant (PCA) II to perform and assist with personal care ADLs/IADLs for participants in the PACE across all delivery platforms, including center, home, or virtually.

The PCA II must be able to perform the duties of a PCA I if assigned and collaborates with IDT members in care planning. The PCA II supports care coordination, aims to maintain or improve participants' independence, mobility, and social engagement, while prioritizing safety

Qualifications

• Certified Nursing Aide education and training required.
• High school diploma required or some college credits preferred.
• Bilingual in English and at least one other language common in service areas.

Responsibilities

• Perform personal care ADLs/IADLs for PACE participants.
• Support care coordination with IDT members.
• Assist PCA I duties when assigned.
