Job summary

Sbhonline is seeking a Personal Care Assistant (PCA) Coordinator in Brooklyn, NY to manage a patient caseload and ensure seamless home care services. The role acts as the main liaison among patients, caregivers, and families, coordinating staffing, addressing concerns, and ensuring regulatory compliance and quality assurance.

The successful candidate will have a high school diploma, fluency in Yiddish, and experience in healthcare coordination or related administrative roles.

Qualifications

• High school diploma required; some college coursework or business education preferred.
• Must be fluent in Yiddish.
• Previous experience in home care, healthcare coordination, scheduling, or a related administrative role is preferred.
• Strong telephone, organizational, and communication skills.
• Excellent attention to detail and the ability to maintain accurate documentation and records.
• Basic computer proficiency, including Microsoft Office and the ability to learn healthcare software systems.
• Strong interpersonal skills with the ability to build positive relationships with patients, families, and caregivers.
• Ability to prioritize multiple tasks and work effectively in a fast-paced environment.
• Demonstrated sound judgment, professionalism, and a commitment to providing outstanding customer service.

Responsibilities

• Serve as the primary point of contact for patients, caregivers, and family members.
• Coordinate caregiver staffing and scheduling based on patient needs and authorized service hours.
• Resolve patient and caregiver concerns promptly and professionally to ensure continuity of care.
• Maintain compliance with regulatory and organizational requirements through accurate documentation, recordkeeping, reporting, and audit readiness.
• Collaborate with internal departments to support payroll, benefits administration, and daily operations.
• Monitor patient cases to ensure services are delivered according to care plans and authorized hours.
• Provide ongoing support, guidance, and communication to patients and caregivers.
• Maintain accurate schedules, case records, and operational documentation.
• Assist with quality assurance initiatives and continuous process improvement.
• Perform additional coordination and administrative duties as assigned.
