Job summary

Hebrew SeniorLife is seeking a Patient Care Associate (PCA) to protect, sustain, and nurture residents in a home-like setting. You will assist with activities of daily living, care for the patient’s environment, and support the care team to ensure safe, high-quality care.

Responsibilities include recognizing patient needs, communicating changes to the nursing team, maintaining infection control, documenting care in the EMR, and participating in ongoing training.

Qualifications

• Long term chronic care PCA CNA certification preferred in good standing on hire or pending nursing school enrollment.
• RSU PCA CNA certification required in good standing on hire and ongoing.
• Fluent English skills, both verbal and written.
• Computer literacy required.
• Flexible and able to work independently and as part of a team.
• Must complete mandatory HSL/HRC educational requirements.

Responsibilities

• Recognize and respond to patients' and families' needs.
• Communicate changes in patient condition to the nursing team.
• Adhere to safe patient handling; maintain no manual lift environment.
• Participate in quality improvement programs.
• Assist with all aspects of personal care per care plans.
• Document care delivered in the EMR.
• Report accidents, incidents, and patient abuse as they occur.
• Observe skin integrity and participate in daily care planning meetings.
