Responsibilities

• Recognize and respond to patient and family needs.
• Communicate changes in patient condition to nursing staff.
• Adhere to Safe Patient Handling practices.
• Participate in Quality Improvement programs.
• Assist patients with personal care per care plans.
• Document care in EMR.
• Report accidents and incidents.
• Report suspected patient abuse.
• Monitor skin integrity.
• Join daily report and care planning meetings.
• Obtain vital signs and basic procedures as directed.
• Follow infection control practices.
• Attend ongoing training and education classes.
• Maintain patient confidentiality per HIPPA.
• Communicate effectively with peers and families.
• Facilitate life enrichment activities.
• Maintain cleanliness of patient rooms and common areas.
• Prepare, serve, and clean up meals and snacks.
• Participate in committee meetings.
• Perform other duties as assigned.
