Responsibilities

  • Serve as the primary point of contact for patients, caregivers, and family members.
  • Coordinate caregiver staffing and scheduling based on patient needs and authorized service hours.
  • Resolve patient and caregiver concerns promptly and professionally to ensure continuity of care.
  • Maintain compliance with regulatory and organizational requirements through accurate documentation, recordkeeping, reporting, and audit readiness.
  • Collaborate with internal departments to support payroll, benefits administration, and daily operations.
  • Monitor patient cases to ensure services are delivered according to care plans and authorized hours.
  • Provide ongoing support, guidance, and communication to patients and caregivers.
  • Maintain accurate schedules, case records, and operational documentation.
  • Assist with quality assurance initiatives and continuous process improvement.
  • Perform additional coordination and administrative duties as assigned.

Personal Care Assistant (PCA) Coordinator

Position Summary

The PCA Coordinator oversees an assigned caseload to ensure the seamless delivery of home care services and an exceptional patient experience. This role serves as the primary liaison between patients, caregivers, and family members by addressing concerns, coordinating staffing needs, and promoting continuity of care.
