Rock Health is seeking a Personal Care Aide to provide direct client care in home settings. You will follow care plans, perform daily tasks, assist with hygiene, mobility, and meals, including special diets, and maintain a clean, safe environment.

Follow care plans, complete daily tasks via EVV or electronic timesheets, assist with personal hygiene, mobility, meal preparation (including special diets), and basic personal care such as bathing, grooming, and oral hygiene. Utilize safety and infection control procedures, maintain a clean and safe environment, and notify supervisors of any issues or changes in client condition. Respect client confidentiality and privacy, and support self-help activities.
High school diploma preferred; ability to read and write; at least 6 months of validated caregiving experience; must be 18 or older. Ability to assist with transfers, use of mobility equipment, and support medication management without administering medications. Strong attention to safety, confidentiality, and client dignity is essential.
Ensure quality care within scope, adhere to care plans, report concerns or unsafe conditions, and collaborate with supervisors. Secondary tasks may include light household chores related to client needs. This role reports to the Home Care Manager or designated staff and involves accountability for client well-being and safety.
