Job summary

Qualifications

• Bachelor's Degree required.
• Minimum 1 year experience in health care marketing and skilled nursing facility admissions and/or hospital case management.
• Strong interpersonal and confidentiality skills are essential.

Responsibilities

• Manage discharges and create open bed resources with bed availability and escalation oversight.
• Coordinate with social workers, nurses, providers, insurers, ElderONE, and external agencies.
• Link residents being discharged with follow-up healthcare services and housing plans.
• Maintain data across databases and develop best practices for project execution.
