Job summary

Humana is seeking a Patient Services Coordinator to schedule visits, coordinate with field staff and physicians, and maintain care continuity for patients. The role also supports daily office operations and staff management.

Responsibilities include editing schedules, handling reassignments, maintaining logs, and assisting with clinical questions. This position requires strong communication and organizational skills with home health experience preferred.

Qualifications

• Must possess excellent communication skills and the ability to interact well with a diverse group of individuals.
• Strong organizational skills and the ability to manage multiple assignments efficiently.
• Experience in home health or similar care coordination environments is preferred.

Responsibilities

• Schedule visits for patients and coordinate with field staff, patients, and physicians.
• Edit schedules and reassign patients when agents are unavailable.
• Maintain the patient hospitalization log and ensure timely communication across teams.
• Process infection control forms and escalate clinical questions as needed.
• Assist with daily office and staff management and participate in case conferences.
