Job summary

Ideal candidate has a Bachelor's in Sociology or Psychology, strong communication, and at least 2 years in hospital or skilled nursing settings.

Qualifications

• Bachelor's degree in Sociology or Psychology or equivalent education/experience.
• Outstanding communication skills (verbal and written).
• Minimum of 2 years' experience in hospital or skilled nursing setting.
• Ability to multi-task, prioritize, and maintain accurate records.

Responsibilities

• Develops a comprehensive social history and assessment of residents for care plans.
• Assists residents and caregivers with transitioning to new environments and orientations.
• Acts as mediator to resolve concerns affecting care, admission, discharge, or treatment.
• Participates in developing individuals' care plans including social services goals.
• Coordinates ongoing care services and attends related meetings.
• Ensures completion of directives and fosters positive relationships with patients and families.
