Job summary

Enhabit Home Health & Hospice is seeking a Scheduling Coordinator to organize patient appointments and coordinate care across branch field clinicians, patients, physicians and caregivers to ensure seamless, high-quality home-based care.

You will manage the on-call notebook and hospitalization logs, supporting clear communication among all stakeholders and helping the care team respond promptly to changes in patient needs.

Qualifications

• Must have a high school diploma or equivalent.
• Must be an LPN/LVN in the state with at least one year of clinical experience, or have 1+ year in home health/hospice/pediatrics within last 24 months and understand staffing/scheduling for home care.
• Must have basic technology skills, including operation of a mobile device.
• Prior home health, hospice or pediatrics experience preferred.
• Valid state driver license, automobile liability insurance, dependable transportation; able to drive in all weather.

Responsibilities

• Schedule patients to branch field clinicians.
• Communicate with field staff, patients, physicians, referral sources, caregivers and other service providers to maintain care coordination.
• Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
