Qualifications

• High school diploma or equivalent required.
• Licensed Practical or Vocational Nurse (LPN/LVN) in current state with at least 1 year of clinical healthcare experience, or at least 1 year of home health, hospice, or pediatric experience in the last 24 months with staffing/scheduling knowledge.
• Basic technology skills including operation of a mobile device.

Responsibilities

• Schedule patients to branch field clinicians.
• Communicate with field staff, patients, physicians, referral sources, caregivers and other service providers to maintain care coordination and continuity of care.
• Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
