Job summary

Reading Hospital, part of Tower Health, seeks a Patient Safety professional to lead RL Solutions reporting and investigations across all sites. The role serves as a resource for patient safety concepts, tools, and methods, guiding Cause Analysis and risk management processes.

The position collaborates with leadership to implement corrective actions, engages frontline staff, and ensures regulatory compliance with MCARE and PA safety authorities.

Qualifications

• Bachelor's degree or equivalent combination of education & experience.
• CPPS certification required within 2 years of employment.
• Clinical experience in patient safety or risk management preferred.
• RN preferred but not required.

Responsibilities

• Supports Tower Health across RL Solutions for reporting and investigating events.
• Advises on Cause Analysis methods to identify causes and contributing factors for safety events.
• Facilitates Failure Modes and Effects Analysis and Apparent Cause Analysis Teams.
• Works with leaders to ensure corrective actions are addressed and improvement monitored.
• Engages frontline staff in the electronic reporting system.
• Advocates for and supports improvement efforts and regulatory compliance.
• Collaborates with the Patient Safety and Risk Management Structure and TH community.
• Assign Harm Scores and standardize use of Harm Scores across the facility.
