Tower Health is seeking a Patient Safety Process Manager for the Pottstown and Phoenixville sites. You will support using RL Solutions for event reporting and investigations, serve as a resource on patient safety concepts, and facilitate analytical teams to identify root causes and corrective actions.

• Bachelor's Degree required or combination of education/experience considered
• Registered Nurse preferred but not required; clinical experience required
• Certified Professional in Patient Safety (CPPS) required completion within 2 years of employment
• CPPS certification prioritized for role

Responsibilities

• Supports Tower Health to utilize RL Solutions for event reporting and investigation across sites
• Engages with leaders to ensure corrective actions are addressed and improvements monitored
• Acts as organizational resource for patient safety concepts, tools and methods
• Facilitates and advises on Cause Analysis, FMEA, and Apparent Cause Analysis teams
• Collaborates with frontline staff and those responsible for event reporting in the electronic system
• Advocates for improvement efforts and regulatory compliance, knowledgeable about RL Solutions and MCARE/DOH expectations
• Assigns Harm Scores and supports standard use of Harm Scores across the facility
• Works with clinical and regulatory teams to ensure safety and quality outcomes

Job Summary

The Patient Safety Process Manager (for Pottstown & Phoenixville):

• Supports individuals across Tower Health to successfully utilize RL Solutions for reporting, investigating events and managing the system across all sites.
• Acts as an organizational resource for patient safety concepts, tools and methods.
• Facilitates and advises on Cause Analysis methods to identify causes and contributing factors for safety events and near misses.
• Facilitates Failure Modes and Effects Analysis and Apparent Cause Analysis Teams.
• Works with key leaders to assure that corrective actions are addressed, and improvement monitored in response to what is learned in the analysis.
• Engages with frontline staff, staff responsible for event reporting and is active in the electronic reporting system.
• Advocates for/supports improvement efforts and acts as a resource for clinical regulatory compliance.
• Performs all the above in collaboration with others in the Patient Safety and Risk Management Structure and others in the TH community as appropriate for the situation. They are knowledgeable about both RL Solutions as well as MCARE and reporting expectations for the DOH and PA Safety Authority.
• Assign Harm Scores and support a process to review and ensure a standard approach to the use of Harm Scores across the facility.
