Tower Health seeks a Patient Safety Process Manager to guide reporting, investigation, and improvement of safety events across its Pennsylvania network. You will collaborate with frontline staff and leadership to promote effective use of RL Solutions and ensure timely corrective actions.

Responsibilities include facilitating risk analyses, coordinating with Risk Management and Compliance, and maintaining Harm Score consistency to meet DOH and PA Safety Authority expectations.

Qualifications

• Bachelor's degree required or equivalent combination of education and experience.
• CPPS certification required within 2 years of employment.
• RN experience preferred but not required.

• Facilitates and advises on Cause Analysis methods to identify causes and contributing factors for safety events and near misses.
• Facilitates Failure Modes and Effects Analysis and Apparent Cause Analysis Teams.
• Works with leaders to ensure corrective actions are addressed and improvement monitored.
• Engages frontline staff and those responsible for event reporting in the electronic system.
• Advocates for improvement efforts and supports compliance with clinical regulatory requirements.
• Assigns Harm Scores and supports standard use across the facility.

Job Summary

Patient Safety Process Manager:

• Supports individuals across Tower Health to successfully utilize RL Solutions for reporting, investigating events and managing the system across all sites.


• Acts as an organizational resource for patient safety concepts, tools and methods.


• Facilitates and advises on Cause Analysis methods to identify causes and contributing factors for safety events and near misses.


• Facilitates Failure Modes and Effects Analysis and Apparent Cause Analysis Teams.


• Works with key leaders to assure that corrective actions are addressed, and improvement monitored in response to what is learned in the analysis.


• Engages with frontline staff, staff responsible for event reporting and is active in the electronic reporting system.


• Advocates for/supports improvement efforts and acts as a resource for clinical regulatory compliance.


• Performs all the above in collaboration with others in the Patient Safety and Risk Management Structure and others in the TH community as appropriate for the situation. They are knowledgeable about both RL Solutions as well as MCARE and reporting expectations for the DOH and PA Safety Authority.


• Assign Harm Scores and support a process to review and ensure a standard approach to the use of Harm Scores across the facility.

Qualifications

Education Requirements

• 4 year Bachelor's Degree - OR - combination of relevant education & experience may be considered in lieu of Bachelor's Degree

Experience

• Registered Nurse preferred by not required


• Clinical Experience Required

Certification and Licensure

• Certified Professional in Patient Safety (CPPS) Required completion within 2 years of employment
