Job summary

LifeBridge Health is seeking a patient navigator to assess patient needs and connect individuals with community agencies and resources based on clinician referrals.

The role conducts initial assessments, gathers relevant data, and reviews referral requests to determine appropriate supports, including documentation of income, medications, and other vital information to enable smooth transitions of care.

Qualifications

• Bachelor's degree in Human Services or related field is required.
• Valid MD Driver's License and clearance by LBH Fleet Safety Department to operate vehicles required.

Responsibilities

• Assess patient needs and capabilities in the context of referral requests by the primary clinician.
• Conduct an initial assessment and gather data relevant to patient needs.
• Gather and document information needed for making patient referrals (e.g., income proof, current medications).
• Coordinate services and resources to meet patients' ancillary needs.
