LifeBridge Health is seeking a Nurse Navigator to identify and manage high-risk patient populations and coordinate care across hospital and community settings. You will educate and support high-utilizer patients while collaborating with a nurse, social worker and community health worker to implement comprehensive plans of care.

The role involves in-person visits across hospital, ED and home settings, strong advocacy, and ongoing quality improvement efforts to improve patient outcomes and care

Competenze

• 1–3 years nursing experience.
• Bachelor's degree in nursing preferred.
• Maryland RN license or eligibility.
• AHA CPR certification.

Mansioni

• Develop and coordinate individualized plans of care with physicians and health care team.
• Assist patients in navigating health care and community support systems.
• Conduct in-person visits at hospital, ED, offices, homes, and by phone.
• Advocate for patients, families, and caregivers to address needs.
• Perform comprehensive nursing assessments to identify strengths and needs.
• Ensure responsiveness to patient/family needs and contribute to quality improvement.
• Collaborate with multidisciplinary team to develop programs for high-risk patients.
• Assist in developing community patient navigation programs.

Formazione

Bachelor's degree in Nursing

Maryland RN license

AHA CPR (Basic Life Support)
