Job summary

Conviva Senior Primary Care in Palm Coast, FL, is seeking a Referral and Scheduling Coordinator to schedule and pre-register patients for exams with external specialists. The role supports a TB screening program and reports to the Medical Referrals Supervisor.

Responsibilities include gathering and communicating preparation instructions, handling referrals within policy, and maintaining accurate records with patients and referring providers.

Qualifications

• Minimum 2 years of hands-on referral healthcare experience in a clinical setting, preferably within the Medicare population
• Proficient with medical terminology and Microsoft Office applications (Word, Excel, Outlook)
• Experience with Electronic Medical Records (EMR)
• Excellent professional communication abilities with patients and colleagues
• TB screening program readiness or willingness to be screened
• Must be passionate about improving consumer experiences

Responsibilities

• Schedule and pre-register patients for exams and procedures with external specialists/provider network
• Gather and communicate preparatory information and instructions to patients and referring providers
• Assist with referrals processing within department policies and quality standards
• Maintain accurate documentation and respond to inquiries in a timely manner
