St. Joseph’s/Candler Health System seeks a Patient Placement Coordinator to manage admissions calls, collect essential demographics, and communicate room assignments to physicians and facilities.
The role supports bed availability by coordinating with charge nurses and Environmental Control to ensure timely patient placement. Preferred candidates hold a High School Diploma and demonstrate strong communication and data entry skills to maintain accurate admission records and efficient transfer
The Patient Placement Coordinator is responsible for taking calls from physician's or designee to schedule an admission, obtain demographic information and communication to the physician offices of the rooms assigned for the incoming patients. The Patient Placement Coordinator also has the responsibility to contact any referring facilities with the notification of rooms assigned in order to expedite the transfer of patients to our facility. The coordinator initiates the patient placement process by identifying an appropriate bed and contacting the charge nurse of the unit to coordinate the room assignment for all patients entering the system. Additional functions include transferring existing patients to different areas, updating patient information in the computer, and communication to appropriate personnel regarding overall census and the availability of beds and prioritizing the need for rooms to be cleaned for Environmental Control. The Patient Placement Coordinator will provide excellent internal and external customer service and maintain a working knowledge of all hospital and departmental policies and procedures and professional standards
