Job summary

ChristianaCare is seeking a Patient Navigator to guide patients through defined care pathways, coordinating appointments, tests, and referrals within the healthcare system. The role emphasizes timely completion of steps, barrier removal, and collaboration with clinical and operational teams.

Minimum education is a high school diploma or GED, with preferred associate or bachelor’s in public health or related field.

Qualifications

• High school diploma or GED required.
• 1 year of experience in patient navigation or related field preferred.
• Experience with health records, referral platforms or scheduling tools preferred.
• Access to a reliable vehicle and a valid driver's license.

Responsibilities

• Guide patients through required sequence of appointments, tests and referrals.
• Coordinate pathway-specific scheduling, reminders and handoffs.
• Track patient status against pathway milestones and follow up as needed.
• Identify barriers to pathway completion and connect to appropriate resources.
• Communicate pathway status and exceptions to the team per escalation protocols.
• Document outreach, milestones, barriers and outcomes in the EHR or system.
