ChristianaCare is seeking a Patient Navigator to guide patients through defined health care pathways and time-limited episodes of care. You will identify barriers, coordinate pathway appointments, and support timely completion of steps in collaboration with clinical and operational teams.

The role requires excellent communication, HIPAA compliance, and the ability to work a flexible schedule, including occasional evenings or weekends.

Qualifications

• High school diploma or GED required.
• Associate or bachelor’s degree in public health, health services, social services, health education, or a related field preferred.

Responsabilités

• Receive and review referrals for assigned clinical or program pathways and confirm that patients meet established navigation criteria.
• Orient patients and families to the pathway, including purpose, steps, responsibilities, and contacts.
• Guide patients through required appointments, tests, referrals, services, or milestones using approved workflow.
• Coordinate pathway-specific scheduling, reminders, preparation instructions, records, forms, and handoffs with clinical and operational teams.
• Track patient status against milestones and follow up on missed or delayed steps.
• Identify barriers (transportation, language access, scheduling, technology, documentation) and connect to resources.
• Communicate pathway status and barriers to the clinical, operational, social care, or program team per escalation protocols.
• Close navigation services when milestones are met and document outcomes accurately in EHR.
• Participate in pathway huddles, training, and quality improvement activities.
