Job summary

Giving Home Health Care is seeking an Outreach Support Specialist (OSS) to manage incoming leads, coordinate patient intakes, and maintain accurate Salesforce records. You will support Outreach Coordinators, communicate professionally with prospective patients and families, and help move qualified leads through the intake process in a fast-paced, mission-driven environment.

Qualifications

• High school diploma or equivalent.
• Strong verbal and written communication skills.
• Professional telephone and customer-service skills.
• Ability to work effectively in a fast-paced environment.
• Strong organizational and time-management skills.
• Ability to manage multiple leads and priorities simultaneously.
• Basic computer proficiency.
• Ability to learn and utilize Salesforce and other company systems.
• Ability to maintain confidentiality and follow HIPAA and company privacy requirements.
• Ability to accept coaching and work collaboratively as part of a team.

Responsibilities

• Lead Management & Patient Follow-Up: respond to leads, contact within 24 hours, document activity in Salesforce, identify qualifying patients.
• Intake Coordination: schedule intakes, confirm appointments, collect demographics and insurance info.
• Salesforce & Documentation: maintain records, review reports, ensure compliance and confidentiality.
• Patient Communication: act as first point of contact, explain next steps, demonstrate empathy.
• Support for Outreach Coordinators: assist scheduling, manage leads from outreach events.
• Phone & Communication Responsibilities: answer calls, use Teams, maintain professional tone.
• Pipeline Development & Re-Engagement: re-engage leads and maintain active pipeline.
• Outreach Event Support: assist events, collect contacts, enter leads in Salesforce.
