Job summary

Giving Home Health Care is seeking an Outreach Support Specialist to manage incoming leads, coordinate intakes, maintain Salesforce records, and support outreach activities. You will ensure timely follow-up, accurate data, and professional patient communication while helping to grow the patient pipeline and coordinate community outreach events.

The role emphasizes empathy, confidentiality, and collaboration with Outreach Leadership, Growth, Clinical, and other teams to enhance the patient

Qualifications

• High school diploma or equivalent.
• Strong verbal and written communication skills.
• Professional telephone and customer-service skills.
• Ability to work in a fast-paced environment.
• Strong organizational and time-management skills.
• Ability to learn Salesforce and other company systems.
• Ability to maintain confidentiality and follow HIPAA and privacy requirements.
• Ability to collaborate as part of a team.

Responsibilities

• Lead management and patient follow-up across multiple sources.
• Coordinate intakes and schedule appointments with accuracy.
• Maintain records in Salesforce and document interactions.
• Provide professional, empathetic patient communication.
• Support Outreach Coordinators with scheduling and lead nurturing.
• Handle phone and digital communications professionally.
• Assist with developing a robust patient pipeline and outreach events.
