Job summary

HarmonyCares is seeking an Outreach Specialist I to perform high-volume outbound calls to schedule in-home visits with a licensed Nurse Practitioner, while also answering inquiries from members and case managers. The role supports enrollment, care coordination, and patient engagement in a remote, nationwide setting.

Qualifications

• High School graduate or equivalent.
• Strong computer skills and proficiency with Microsoft Office and internet-based applications.
• Self-motivated and able to work independently.
• Demonstrated success in quickly building telephonic relationships with prospective customers.

Responsibilities

• Place a high volume of outbound calls to secure appointments with an NP in the member's home.
• Answer incoming calls and return voicemails promptly.
• Follow talking points and protocols for member outreach calls.
• Schedule visits efficiently using the scheduling software.
• Maintain HIPAA guidelines on every call and verify demographics and insurance.
