Qualifications

• Review Epic data and value-based care metrics to identify opportunities for primary care practices.
• Translate population health metrics into actionable next steps for practice teams.
• Engage with practice managers to review performance and develop improvement strategies.

Responsibilities

• Reviewing Epic data and value-based care metrics to identify opportunities and actionable targets for primary care practices.
• Meeting with practice managers to review performance, discuss action items, and develop strategies to improve outcomes.
• Attending monthly partner meetings and preparing presentations, including annual wellness visit data.
• Supporting practice operations by implementing workflow changes and helping teams adopt sustainable processes.
• Providing Epic education and training to providers and staff as needed.
• Serving as a liaison between the Medical Group, primary care practices, and physician leadership by sharing information and gathering feedback.
• Developing reports with practices, providers, and staff to understand needs and monitor progress.
• Assisting with patient outreach and efforts to close gaps in care.
• Identifying low-performing areas and serving as an escalation point to help address barriers and improve value-based care performance.
• Reviewing upcoming value-based programs and coordinating participation with the Primary Care Service Line.
