Job summary

RWJBarnabas Health is seeking a Patient Navigator to support Primary Care practices in achieving improved value-based performance. The role leverages Epic data and population health metrics to identify gaps, develop actionable strategies, and educate practice teams.

Travel across Jersey City/Hoboken area is required to coordinate and implement workflow changes. The position is salaried, full-time, with day shifts and 40 hours per week, working with the Administration-Primary Care team to align

  • Reviewing Epic data and value-based care metrics to identify opportunities and actionable targets for primary care practices.
  • Meeting with practice managers to review performance, discuss action items, and develop strategies to improve outcomes.
  • Attending monthly partner meetings and preparing presentations, including annual wellness visit data.
  • Supporting practice operations by implementing workflow changes and helping teams adopt sustainable processes.
  • Providing Epic education and training to providers and staff as needed.
  • Serving as a liaison between the Medical Group, primary care practices, and physician leadership by sharing information and gathering feedback.
  • Developing reports with practices, providers, and staff to understand needs and monitor progress.
  • Assisting with patient outreach and efforts to close gaps in care.
  • Identifying low-performing areas and serving as an escalation point to help address barriers and improve value-based care performance.
  • Reviewing upcoming value-based programs and coordinating participation with the Primary Care Service Line.

The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

As part of RWJBarnabas Health Medical Group, the Patient Navigator plays an instrumental role in supporting medical group practices in achieving improved performance across various Value-Based Programs. The Patient Navigator works directly with the Primary Care Service Line, practice leadership, and physician leadership teams to support the efficient delivery of value to patients and physicians while advancing the RWJBH “One System – One Family” model.

The Patient Navigator serves as a key practice-facing resource responsible for aligning transformation activities, identifying opportunities to improve value-based performance, supporting workflow redesign, and facilitating the implementation and dissemination of initiatives across Medical Group practices. This role uses data and practice insights to identify gaps in care and performance, develop actionable strategies, educate practice teams, and support sustainable improvements in clinical and operational workflows.

As a Patient Navigator, a typical day might include the following:
  • Reviewing Epic data and value-based care metrics to identify opportunities and actionable targets for primary care practices
  • Meeting with practice managers to review performance, discuss action items, and develop strategies to improve outcomes
  • Attending monthly partner meetings and preparing presentations, including annual wellness visit data
  • Supporting practice operations by implementing workflow changes and helping teams adopt sustainable processes
  • Providing Epic education and training to providers and staff as needed
  • Serving as a liaison between the Medical Group, primary care practices, and physician leadership by sharing information and gathering feedback

  • Schedule: Day shift, Monday through Friday 8AM-4:30PM or 8:30AM-5PM, Full-Time 40 hours per week

To be consider for this opportunity, you must have an Associate’s Degree in healthcare administration, business, or a related field and 3–5 years of experience in practice management, healthcare operations, or a related role. Knowledge of medical terminology and HIPAA requirements is also required. Travel required. This role requires up to 75% travel throughout the Jersey City/Hoboken areaand a valid New Jersey driver’s license with dependable transportation. A bachelor’s degree in healthcare administration, business, or a related field is preferred. Epic, population health, physician practice experience, and LEAN/Six Sigma certification are also preferred.
