As part of RWJBarnabas Health Medical Group, the Patient Navigator plays an instrumental role in supporting medical group practices in achieving improved performance across various Value-Based Programs. The Patient Navigator works directly with the Primary Care Service Line, practice leadership, and physician leadership teams to support the efficient delivery of value to patients and physicians while advancing the RWJBH "One System - One Family" model.
The Patient Navigator serves as a key practice-facing resource responsible for aligning transformation activities, identifying opportunities to improve value-based performance, supporting workflow redesign, and facilitating the implementation and dissemination of initiatives across Medical Group practices. This role uses data and practice insights to identify gaps in care and performance, develop actionable strategies, educate practice teams, and support sustainable improvements in clinical and operational workflows.
To be consider for this opportunity, you must have an Associate's Degree in healthcare administration, business, or a related field and 3-5 years of experience in practice management, healthcare operations, or a related role. Knowledge of medical terminology and HIPAA requirements is also required. Travel required. This role requires up to 75% travel throughout the Jersey City/Hoboken area and a valid New Jersey driver's license with dependable transportation. A bachelor's degree in healthcare administration, business, or a related field is preferred. Epic, population health, physician practice experience, and LEAN/Six Sigma certification are also preferred.
