Job summary

RWJBarnabas Health in New Jersey seeks a Patient Navigator to support value-based care across Primary Care practices. You will analyze Epic data, coordinate with practice leaders, and drive workflow improvements to elevate patient outcomes.

Day-to-day duties include reviewing metrics, meeting managers, presenting at partner meetings, training staff on Epic, and serving as a liaison to physicians. This role requires 3–5 years in healthcare operations and travel up to 75%.

Qualifications

  • Associate's degree in healthcare administration, business, or related field required.
  • 3-5 years of experience in practice management, healthcare operations, or related role.
  • Knowledge of medical terminology and HIPAA requirements.
  • Travel up to 75% in the Jersey City/Hoboken area.
  • Epic, population health, LEAN/Six Sigma certification preferred.

Responsibilities

  • Identifying low-performing areas and addressing barriers to improve value-based care.
  • Reviewing upcoming value-based programs and coordinating participation.

As part of RWJBarnabas Health Medical Group, the Patient Navigator plays an instrumental role in supporting medical group practices in achieving improved performance across various Value-Based Programs. The Patient Navigator works directly with the Primary Care Service Line, practice leadership, and physician leadership teams to support the efficient delivery of value to patients and physicians while advancing the RWJBH "One System - One Family" model.

The Patient Navigator serves as a key practice-facing resource responsible for aligning transformation activities, identifying opportunities to improve value-based performance, supporting workflow redesign, and facilitating the implementation and dissemination of initiatives across Medical Group practices. This role uses data and practice insights to identify gaps in care and performance, develop actionable strategies, educate practice teams, and support sustainable improvements in clinical and operational workflows.

As a Patient Navigator, a typical day might include the following:
  • Reviewing Epic data and value-based care metrics to identify opportunities and actionable targets for primary care practices
  • Meeting with practice managers to review performance, discuss action items, and develop strategies to improve outcomes
  • Attending monthly partner meetings and preparing presentations, including annual wellness visit data
  • Supporting practice operations by implementing workflow changes and helping teams adopt sustainable processes
  • Providing Epic education and training to providers and staff as needed
  • Serving as a liaison between the Medical Group, primary care practices, and physician leadership by sharing information and gathering feedback
  • Developing reports with practices, providers, and staff to understand needs and monitor progress
  • Assisting with patient outreach and efforts to close gaps in care
  • Identifying low-performing areas and serving as an escalation point to help address barriers and improve value-based care performance
  • Reviewing upcoming value-based programs and coordinating participation with the Primary Care Service Line

  • Schedule: Day shift, Monday through Friday 8AM-4:30PM or 8:30AM-5PM, Full-Time 40 hours per week

To be consider for this opportunity, you must have an Associate's Degree in healthcare administration, business, or a related field and 3-5 years of experience in practice management, healthcare operations, or a related role. Knowledge of medical terminology and HIPAA requirements is also required. Travel required. This role requires up to 75% travel throughout the Jersey City/Hoboken area and a valid New Jersey driver's license with dependable transportation. A bachelor's degree in healthcare administration, business, or a related field is preferred. Epic, population health, physician practice experience, and LEAN/Six Sigma certification are also preferred.
