Job summary

RWJBarnabas Health is seeking a Coordinator Clinical to join the Population Health team in New Jersey. You will work across post-acute facilities to coordinate care, ensure timely discharge planning, and support transitions to the next site of care.

The role requires traveling throughout New Jersey and collaborating with a multidisciplinary team. The position is salaried with a day shift, Monday through Friday, and offers benefits as part of RWJBarnabas Health’s total rewards package.

Qualifications

• 3–5 years’ experience managing relationships between acute and post-acute facilities
• Post-acute care setting experience required
• Ability to travel to post-acute and hospital sites
• Strong organizational skills with ability to prioritize multiple tasks
• Strong leadership to achieve buy-in and collaboration
• Willingness to travel throughout New Jersey

Responsibilities

• Perform initial and ongoing assessments of patient condition in post-acute settings
• Develop and evaluate care plans with input from patient/family
• Participate in multidisciplinary rounds to create collaborative care plans
• Coordinate discharge planning and follow-up with community providers
• Educate staff on patient/family education needs
• Document patient responses and need for skilled intervention
• Provide resources and education on readmissions to post-acute facilities
• Serve as a resource for post-acute facilities
