Job summary

JWCH Institute, Inc. is seeking a Patient Navigator in Palmdale, CA to welcome patients, assess needs, and guide them through administrative and support services.

You will work across front desk and clinical teams, communicating clearly and assisting with forms, appointments, and transportation coordination. Requirements include at least 1 year of front desk experience, English/Spanish bilingual ability, a valid CA driver’s license, and willingness to travel between sites.

Qualifications

• Minimum 1 year of front desk or receptionist experience.
• Bilingual proficiency in English and Spanish is highly desirable.
• Valid CA Driver’s License with clean driving record.
• Willingness to travel between sites as required.

Responsibilities

• Welcome clients, visitors, and the general public in a friendly, professional manner.
• Engage with clients to identify informational needs and coordinate with healthcare teams.
• Assist clients with forms and navigating administrative processes.
• Provide information on on-site services and health coverage options; refer to Enrollment Specialists.
• Act as liaison between clients and medical staff for smooth information exchange.
• Coordinate updates from front/back office staff to ensure accurate information flow.
• Travel to different sites as required and participate in safety/compliance trainings.
