Job summary

Jefferson Health in Voorhees, NJ seeks a Patient Navigator to act as a liaison for patients, coordinating services with physicians and staff, scheduling appointments across departments, and following up to assure compliance. The Patient Navigator supports the Jefferson Experience with prompt and courteous care for all patients and visitors.

The role emphasizes HIPAA compliance and collaboration, including triaging calls, coordinating inpatient care, and maintaining community resource lists.

Qualifications

• 3 years of experience in a medical office.
• Must be able to communicate courteously with patients and/or visitors.
• Willingness to provide and support a positive Jefferson Experience for all patients and/or visitors.
• Leadership skills to include proactivity and problem solving.
• Ability to establish and develop positive and collaborative relationships.

Responsibilities

• Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
• Schedules patients for all visits undergoing future care
• Administers screenings and intake assessments as needed
• Monitor treatment plan adherence
• Triage patient phone calls
• Coordinate inpatient care as needed
• Develop, maintain, update community resource list
• Greets and interacts with patients/visitors in a professional manner, on phone or in person.
• Adheres to HIPAA privacy and security requirements. Ensures compliance with all applicable federal, state, and local regulatory standards.
• Other duties as requested or assigned.
