Job summary

Jefferson Health in Voorhees, NJ seeks a patient navigator RN to provide trauma-informed, patient-centered medical case management across the HIV care continuum, ensuring timely access to services, treatment adherence, and continued engagement in care.

As RN Medical Case Manager, you will lead an interdisciplinary team, coordinate access to medical care, labs, medications and supportive programs, educate on HIV adherence, U=U, safer sex, support rapid treatment start, and advocate for clients

Qualifications

• RN with BLS certification and HIV care experience.
• Experience in patient navigation or care coordination preferred.
• Strong communication and advocacy skills.

Responsibilities

• Interact with co-workers and staff in alignment with Jefferson values.
• Conduct medical intake assessments for new HIV diagnoses within 24 hours or for transfers.
• Coordinate access to medical care, labs, medications, and supportive services.
• Educate patients on HIV treatment adherence, U=U, and safer sex.
• Support rapid start of HIV treatment and advocate for clients.
• Engage clients every 6 months and participate in chart scrubbing and care teams.
• Plan and execute long-term injectables for prevention and treatment.
