Job summary

Kaniksu Community Health in Sandpoint, ID is seeking a Patient Navigator/Behavioral Health Referral Coordinator to provide an outstanding patient experience while handling essential clinical and administrative tasks in a fast-paced clinic.

A typical day includes guiding patients through medical and behavioral health services, managing referrals, and documenting activities in the EHR to support PCMH care coordination. High school diploma required; bilingual English/Spanish a plus.

Qualifications

• High school diploma or GED.
• Strong organizational skills with ability to track multiple referrals and deadlines.
• Excellent verbal and written communication; empathy and trauma-informed approach.
• Ability to maintain professional boundaries with patients in distress.
• Proficiency with EHR, Microsoft Office, phones, fax, and electronic referral platforms.
• Ability to work independently and as part of an interdisciplinary team.
• Experience in healthcare, behavioral health, social services, or customer-facing patient support.
• Experience with referral management, prior authorizations, care coordination, or case management; preferred in FQHC or community health setting.
• Basic understanding of medical and behavioral health terminology.
• Bilingual English/Spanish is a plus.

Responsibilities

• Serve as a welcoming, consistent point of contact for new patients across departments.
• Help patients navigate and connect across KCH departments.
• Escalate signs of crisis to clinical staff per protocol.
• Receive and process behavioral health referrals; verify and route to clinicians.
• Track referrals in the EHR/referral log from receipt to completion.
• Follow up to confirm appointments and referral closures.
• Communicate referral status to patients, providers, and care teams.
• Maintain relationships with external behavioral health partners.
• Schedule intake and follow-up appointments in coordination with clinic staff.
• Document all patient contacts and referrals in the EHR.
• Obtain and manage releases of information; protect HIPAA compliance.
• Assist in running referral tracking reports for quality improvement.
