Job summary

Kaniksu Community Health in Sandpoint, ID is seeking a Referral Coordinator – Behavioral Health to provide excellent patient service and handle referrals in a fast-paced PCMH team setting.

Under supervision of nursing leadership, you will schedule onboarding, ensure counseling referrals are connected to the Behavioral Health Administration Team, and support process improvements within the PCMH framework.

Qualifications

  • High school diploma or GED or equivalent.
  • 2+ years of experience in a medical environment with medical terminology knowledge.
  • Experience in a behavioral health clinic is advantageous but not required.
  • Proficient computer skills: word processing and Excel; EHR familiarity preferred (AthenaOne).
  • Must be highly organized, detail-oriented, process-minded, and able to follow directions.
  • Excellent verbal and written communication; maintain confidentiality and HIPAA compliance.

Responsibilities

  • Manage all internal and external referrals for Behavioral Health Services with proper documentation.
  • Contact patients to schedule onboarding and appointments with providers.
  • Ensure onboarding and initial evaluations are completed and routed to the appropriate team.
  • Follow defined processes and report improvement opportunities monthly.

The Kaniksu Community Health Referral Coordinator – Behavioral Health is not just responsible for providing excellent customer service. They are committed to providing an excellent patient experience while performing essential clinical or administrative work in a fast-paced, productive environment. YOU ARE an essential team member charged with assisting patients in managing their care at Kaniksu Community Health.

Under the direction of the KCH Director of Nursing, Manager of Patient Experience, and as a member of the Kaniksu Community Health Patient Centered Medical Home (PCMH) Team, a typical day of a Referral Coordinator – Behavioral Health might include:

  • Manage all internal and external referrals for KCH Behavioral Health Services (including psychiatric medication management and counseling services). Ensure all referrals are properly documented in the EMR system and proper tracking is in place.
  • Upon receipt and submission of a psychiatric medication management referral, contact all patients to schedule and perform their Behavioral Health Onboarding and scheduling with the appropriate provider.
  • Upon receipt and submission of a counseling referral, contact all patients to ensure each patient has had their Behavioral Health Onboarding and initial evaluation performed and that the patient is connected to the Behavioral Health Administration Team for discussion of services and scheduling.
  • Ensure that defined processes are followed; look for process improvement opportunities to report to the PCMH team on a monthly basis.
Experience Needed to Land this Gig:
  • High school diploma or general education degree (GED); or equivalent combination of education and experience.
  • 2+ years' experience in a medical environment with knowledge of medical terminology.
  • Experience in a behavioral health clinic or similar would be advantageous, but not essential.
  • Computer skills to include proficiency in word processing; Excel (building and manipulating spreadsheets); strong technical aptitude with the ability to learn new programs quickly; previous experience using an Electronic Health Record preferred, with strong preference for AthenaOne knowledge.
  • Must be highly organized and detail-oriented.
  • Must be process-minded and able to follow precise directions.
  • Advanced, extensive, and documented experience in general office administration and office functions.
  • Excellent oral and written communication skills.
  • Ability to maintain corporate confidentiality and strict HIPAA compliance.
