Base pay range

$21.50/hr - $22.47/hr

Position Summary

We are seeking a dedicated, professional full-time Patient Navigator for our Manhattan location. The ideal candidate is personable, works well with diverse populations, communicates effectively with medical providers and support staff, and has good attention to detail. The Patient Navigator provides field-based case management services to patients enrolled in the HIV care coordination program, serving as the main point of contact, building strong relationships, and helping patients stay engaged in prevention and treatment services. The role involves removing barriers to care by identifying resources and assisting patients in navigating healthcare systems. The Patient Navigator collaborates closely with the care team to ensure optimal health outcomes.

Job Responsibilities
  • Provide direct services to individuals living with HIV/AIDS under the supervision of the Care Coordinator.
  • Execute tasks related to medical and support service plans, including accompanying patients to appointments, coaching, health education, entitlements reassessment, and coordinating logistics such as transportation and childcare.
  • Perform field-based tasks: observe and record patient self-administration of ARVs, assess and report side-effects, and deliver services at home, in community-based organizations, or other convenient locations.
  • Maintain client records and documentation according to protocols.
  • Participate in quality assurance and continuous quality improvement activities.
  • Represent the agency at various venues to promote services.
  • Perform other duties as assigned.
Qualifications
  • Associate’s degree in human services or social work, with at least 12 months experience in patient navigation or a similar field.
  • Certification as a peer worker by the NYS AIDS Institute Peer Certification, case management certification by CUCS, or at least 90 hours of training equivalent to those required by NYS Peer Worker.
  • Strong socio-cultural identification with the target population or experience working with this group.
  • Fluency in one or more Asian languages preferred.
  • Excellent written and verbal communication skills in English required.
  • Proof of immunization or immunity to certain communicable diseases and TB testing is required, in compliance with NYC DOHMH, NYSDOH, and OSHA standards.
