Responsibilities

• Ensures compliance with state, federal, and Joint Commission referral/intake requirements.
• Provides insight and assistance with improved work methods and procedures for admission per policy.
• Establishes and maintains positive relationships with current and potential referral sources.
• Ensures seamless transition of patients to home care and coordinates care with multiple service providers.
• Oversees insurance verification and authorization processes to maximize reimbursement.
• Builds and monitors community and customer perceptions of Sacred Heart Home Health as a high-quality provider.
• Gathers, collates, and reports referral statistics including key trends.
• Maintains knowledge of Home Health contractual relationships and ensures admissions per contract provisions.
• Assists referral sources in accessing community resources when services are not provided.

**I****.****JOB SUMMARY**The Intake Coordinator is responsible for managing all aspects of the patient intake process including communicating directly with patients and families, data entry, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, facilitating a daily intake stand up meeting and managing the insurance verification and authorization processes.**II. DUTIES & RESPONSIBILITIES**1. Directs all daily patient referral and intake operations including providing assistance with oversightof the establishment and implementation of intake policies.2. Ensures compliance with all state, federal, and Joint Commission referral/intake regulatoryrequirements.3. Provides insight and assistance with the implementation of improved work methods and proceduresto ensure patients are admitted in accordance with policy.4. Establishes and maintains positive working relationships with current and potentialreferral sources.5. Ensures seamless transition of patients to home care by assisting with oversight of patient educationand preparation for home care, plan of care initiation and coordination of care with multiple service providers.6. Ensures maximum third-party reimbursement through direct oversight of insurance verification andauthorization processes.7. Builds and monitors community and customer perceptions of Sacred Heart Home Health as a high-quality provider of services.8. Gathers, collates, and reports referral statistics including key customer referral trends.9. Maintains comprehensive working knowledge of Home Health contractual relationships and ensuresthat patients are admitted according to contract provisions.10.
