Job summary

Prismhr Hire in Phoenix is seeking a detail-oriented, patient-focused intake assistant to support a busy medical practice's front end. You will gather and verify patient information, assist with scheduling, and help ensure a smooth intake experience for patients and care teams.

Responsibilities include collecting demographics and insurance data, reviewing documents, maintaining accurate EMR entries, supporting patient flow, and providing clear, empathetic communication.

Qualifications

• High school diploma or equivalent required; healthcare admin education preferred.
• Prior experience in a medical office or customer service role is strongly preferred.
• Strong attention to detail and accuracy in data entry and documentation.
• Excellent interpersonal and communication skills are essential.
• Ability to manage multiple tasks in a fast-paced environment.

Responsibilities

• Serve as the first point of contact for patients completing the intake process.
• Collect, review, and enter patient demographic, insurance, and medical information accurately.
• Verify forms, documents, and records for completeness and consistency.
• Coordinate with clinical and administrative staff to support patient flow.
• Assist with scheduling and follow-up communications as needed.
• Maintain confidentiality and comply with privacy and security standards.
• Provide instructions on navigating the practice website, portal and patient apps.
