Job summary

UNIFI Healthcare is seeking a Physical Therapist to evaluate patients, develop individualized care plans, and deliver hands-on treatment to help patients reach meaningful goals. • Graduate of a CAPTE/APTA-accredited PT program.
• Active PT license (or eligibility) in the state of hire.
• BLS certification (or willingness to obtain).
• A patient-first mindset and strong communication.

Responsibilities

• Evaluate patients and create individualized plans of care
• Deliver hands‑on treatment and progress patients toward meaningful goals
• Partner with a supportive, multidisciplinary team (case managers, respiratory therapy, authorizations)
• Keep documentation clear and timely so care stays coordinated and uninterrupted
• Help create a calm, encouraging environment where patients feel safe to try, rebuild, and improve
