Job summary

UNIFI Healthcare is seeking a dedicated Physical Therapist to evaluate patients and design individualized care plans for home-based therapy. You will deliver hands-on treatment, monitor progress toward goals, and collaborate with case managers and respiratory therapy to ensure seamless care.

The role emphasizes patient-centered care in a supportive, multidisciplinary environment. A CAPTE/APTA-accredited program completion and an active license in the hire state are required, with BLS

Qualifications

• CAPTE/APTA-accredited PT program required or in progress.
• Active PT license (or eligibility) in the state of hire.
• BLS certification or willingness to obtain within reasonable time.

Responsibilities

• Evaluate patients and create individualized plans of care.
• Deliver hands-on treatment and progress patients toward meaningful goals.
• Partner with a multidisciplinary team (case managers, respiratory therapy, authorizations).
• Keep documentation clear and timely for coordinated care.
• Create a calm, encouraging environment where patients feel safe to try, rebuild, and improve.
