Job summary

Jefferson Health is seeking a Patient and Family Centered Care Coach to lead staff training and coaching on communication, empathy, and service excellence. You will use data-driven insights to improve patient experience and promote a culture of compassion and accountability.

The role involves educating teams, facilitating workshops, analyzing patient feedback data, and partnering with leaders to implement targeted action plans across clinical and non-clinical areas.

Qualifications

• Bachelor's degree in a related field is required.
• 3 years healthcare system/organizational improvement, patient experience, and training experience.

Responsibilities

• Educates colleagues on patient-experience projects and programs and creates curricula.
• Facilitates workshops on patient-centered care, service recovery, communication, and empathy.
• Analyze patient experience data (HCAHPS, Press Ganey) to identify trends and opportunities.
• Collaborates with department leaders to create and execute action plans for care improvement.
• Embed patient-centered principles into daily operations and advocate for patient perspectives.
• Manage accountability and sustainability reporting tools and systems.
• Model behaviors through rounding, shadowing, and feedback; assist with service recovery.
