Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required.
• Knowledge of patient- and family-centered practice and collaboration with care partners.
• Cultural awareness of diverse backgrounds and respectful communication.
• Effective communication concepts and partnership with patients and families.
• Appropriately and respectfully delegates to other team members.
• Patient assessment and care knowledge and ability to implement a plan.
• Education of patients and designated care partners on condition and treatment.
• Discharge planning and follow-up care coordination.
• Health information documentation in written or electronic form.
• Active RN license required in state of practice.

Responsibilities
