Job summary

Sage Health is seeking a PRN Patient Experience Associate / Care Coordinator to support front desk operations and care coordination across its medical centers. The role serves as the first contact for patients, handling check-in, insurance verifications, referrals, and appointment coordination.

The position requires flexibility to cover centers within the Baltimore Market and to support care teams. Strong organization, communication, and patient-centered service are essential.

• Experience with Microsoft Office Word, Outlook, and Excel.
• Full COVID-19 vaccination (including boosters) an essential requirement.

Responsibilities

• Greet patients and guests upon arrival and assist with check-in/check-out in the EHR.
• Collect co-pays and verify insurance prior to appointments.
• Answer and triage calls, schedule reminders and reschedules.
• Maintain patient confidentiality and accurate EHR data.
• Scan/index documents and support front desk workflows.
• Coordinate referrals and appointment details with patients and providers.
• Process insurance authorizations and open referrals.
• Notify PCPs of missed appointments and support rescheduling.
• Coordinate surgical clearance documentation as needed.
