Job summary

Sage Health is hiring a PRN Patient Experience Associate / Care Coordinator for the Baltimore Market. This dual-role position covers front desk operations and care coordination, ensuring a positive patient experience, handling referrals, insurance verifications, and appointment coordination across multiple centers.

The ideal candidate is organized, customer-focused, and capable of managing multiple tasks in a busy medical office. COVID-19 vaccination is required where applicable.

Qualifications

• Office/clerical/customer service experience in a fast-paced environment.
• Professional demeanor and clear communication at all times.
• Highly organized with attention to detail and ability to manage competing priorities.
• Responsible and proactive problem solving with accountability.
• Experience with Microsoft Office (Word, Outlook, Excel).
• Vaccination against COVID-19 is required where applicable.

Responsibilities

• Greet patients and guests; manage check-in/check-out in EHR.
• Assist with paperwork and update patient information in EHR.
• Collect co-pays and verify insurance prior to appointments.
• Answer and triage calls; schedule and reschedule appointments.
• Coordinate referrals and insurance authorizations.
• Support care coordination by scheduling appointments and preparing prep instructions.
• Maintain patient confidentiality and accurate documentation.
• Collaborate with clinical team to ensure smooth office operations.
