Upperline Health is seeking a Practice Success Coordinator to educate patients about Upperline Plus and guide enrollment during in-person visits at partner practices. This role requires daily presence in clinics within the assigned region.

The position emphasizes customer service, clear communication, teamwork, HIPAA compliance, and comfort using iPads and Microsoft Office to document and coordinate care.

Qualifications

• High school diploma or equivalent required.
• Minimum 2 years in customer service or patient-facing roles.
• Experience working with older adults is a plus.

Responsibilities

• Educates patients about the Upperline Plus program and enrollment.
• Clarifies benefits to patients during check-in or check-out.
• Collaborates with clinic staff to meet enrollment goals and streamline workflows.
• Represents Upperline’s culture and maintains patient trust and confidentiality.
